Aesthetic correction of reverse smile line
22 years old patient came to our practice with complains on her front teeth. One of the crowns fell down few weeks ago. Circular caries was detected but the dentist was just giving first aid and fixed the crown with temporary cement. After RTG examination we diagnosed radiolucent areas on every teeth in the upper IKS on the roots next to the crown margins. From the intraoral examination we found existing crowns without exact margins placed subgingival with traumatisation of the marginal gingiva. Gingiva is hypertrophic and inflamed. After removing the old crown we found caries on teeth 13, 12, 11, 22, 23. Caries was cleaned and cores were repaired. We reinforced cores with posts on the position 13, 22 and we change the old fashioned preparation in shoulder preparation in order to make exact margins of the new crowns and avoid irritation of the gingiva. From the smile analyses we can see reverse smile line. We tried to correct this smile line just with intervention on the upper IKS by designing longer first incisors and shorter canines. Results are not ideal because smile line is falling down also in TKS, but we made compromise to leave it like this because the patient is very young. We reconstructed all other decays conservatively with fillings.
