Bichectomy
Bichectomy or facial lipectomy is the surgical procedure in which we seek to improve facial features by creating the optical illusion of having a slightly longer and thinner face, thus improving the aesthetic appearance and facial harmony. The bachectomy consists of extracting the Bichat bags from each cheek (between 5 and 8 cc of adipose tissue for each bag) through small incisions, made on the inner side of the mouth. It is done through an incision at 0.5 cm. of the gingivo-labial superior sulcus, at the height of the second premolar, extending 2 cm. posterior parallel to the sulcus. Then, a roman scissors is entered, directing the point towards the temporo-mandibular angle; The fibers of the buccinator muscle are disrupted and with a slight pressure on the cheeks, the Bichat Ball is quickly exposed with a thin capsule, different from that of the lipomas and a bright coloration. Sometimes, a small glass must be coagulated. Each cavity is capped with a dry gauze that is removed at the end of the intervention.
