salam Alikum Good morning dear colleagues. this case was my first chance to use Compressive implants. I had couple only of drills and couple of compressive screws. The case of a 72 years old male with atrophic lower ridge bilateral in posterior area. He is using anticoagulants to previous cardiac stent procedure. The patient complained of unstable lower acrylic denture, where he wanted to place few implants to add more stability. Topical and local anesthesia were applied. Implants sites preparation were prepared used each implant drill with corresponding diameter and then, the corresponding compressive screw. Drilling sites were determined according to CBCT and by use of drilling tool that can determine the space needed for each implant (shown in photos). Implants were placed after each site preparation. Insertion was accomplished by use of torque wrench. Torque exceeded 50 Ncm with each implant. Angulations were adjusted for desired parallelism by use of torque wrench and insertion tool, by applying pressure with thumb on head of wrench and bending by use of other fingers holding the wrench handle. Impression transfers were installed on top of implants. Impression was taken by use addition silicone. Implant analogues were inserted into transfers after impression taking, then impression was poured by use of type 4 stone and sent to lab. An immediate lower denture was fabricated with simple holes corresponding to the implants sites and retaining it in place. Denture was delivered to patient after 72 hours.
