Removing of „autobroken” instrument with the help of home made „loop” . I had a conversation with Cliff Ruddle in London, and he told me: At the end of the year i do not remember the hundreds of sussesfull cases. I do remember clearly the few cases, when i made mistake, or broke an instrument. This tooth (27) was referred to me for RCT treatement. Despite of the calcification, orifices were easily discovered.Capitulation with 0.10 was uncomplicated. During shaping, i broke a Protaper Next X1 in the middle third. First i tried to remove with US and tryed to bypass. This trials ended up with pushing the instrument apically. Than i tried with home made loop. (Made of 27G needle and 0.05 wire) I was able to pull it out into the pulp chambre. After proper cleaning, obturation was carried out. But what had happened? Why has this instrumet broken? I treat hundreds of rootcanals a year, but usually i brake an instrument once in a year. X1 was single used. Buti t was friday evening, ir was last patient after many RCT before. I become very calm after capitulation, i tought i got it, and soon i will go home. Hurried a bit, and used X1 fast. Not took enough care of coronal shaping. It was a second molar with limited opening, and it happened…… But... All is well, that ends well! Shaping: Kerr: 8, 10,15 Protaper SX, Next X1, F1,25/02, 30/02 US: NSK Varios 370: E4d, E8, Satelec Acteon Cleaning:NaOCl 5.25 %, Endoactivator,,US liquid EDTA 15% Obturation: WVC (Sytem B Obtura III, AH+, Buchanan plugers)
