Trust your apex locator Radiographs are a great guide to where the apical foramen is but if you are experienced with the apex locator it's more likely to be correct! The master cone radiograph of tooth 36 (which is the length the apex locator confirmed) looks short! But when you check the post op radiograph is spot on! Radiographs are a helpful tool but tell you very little about where the apical construction is.

Should I trust my apex locator reading ???

When canals are calcified!!!! Tooth 36 had asymptomatic apical periodontitis (first radiograph) Patient requested single visit rct for medical reasons. A very long 2 hour appointment and all canals MB ML D canals located through the existing crown. MB and ML= X2 and D=X3 Irrigation sodium hypo 6% EDTA 15% GP and sealer used to fill canals (2nd radiograph) look at the lateral anatomy associated with the periapical lesions which encompass the root end! 3rd radiograph 13 month review and healing is underway. Despite small apical sizes healing is going well and the patient has good dentine remaining! #workinglengthmoreimportantthanworkingwidth

Working length is a priority and getting the irrigants to reach the length

This IS what team work is all about! The referring dentist placed an ortho band and sent me tooth 46 to perform retreatment on. Tooth 46 had Symptomatic apical periodontitis. This was a carrier based root treatment and I was able to remove the carriers by bypassing them and using the braiding technique. After filling the canals of tooth 46 I suggested that the dentist remove the ortho band and restore tooth 46 with a direct cuspal coverage restoration (which they did really well). Then I wanted the patient to return for the review 8 months after competition of the root filling. So the final radiograph shows the healing after 8 months! Now ready for the dentist to do the crown for tooth 46.

Good work between the specialist and the general dentist. If we support each other then the patient benefits.
